Hi, I’m Blake—Nurse Practitioner and founder of Level-Up APRN PLLC. My goal is simple: to be the kind of provider I would want for my own family. I bring a broad and deeply rooted background in healthcare to every visit, with a focus on holistic, patient-centered care.

I began my journey in medicine as a U.S. Army medic, working in clinic, hospital, and field settings. That commitment continued through years as a critical care paramedic, and then into emergency and critical care nursing. During my Nurse Practitioner training, I completed diverse clinical rotations in primary care, pediatrics, women’s health, behavioral health, and concierge medicine—all of which shaped the practice I run today. Currently, I serve patients across primary care, post-acute rehab, hospice, and palliative care, in addition to teaching future healthcare professionals.

My passion lies in preventative care, education, and improving quality of life—not just treating illness, but helping patients live fuller, healthier lives. At Level-Up, we believe in the provider-patient relationship: you're not just receiving care—you’re partnering in it. As I often say, we're the “Guardians of Your Well-Being.”

When I'm not seeing patients, you’ll probably find me at Disney with my spouse or out with our rescue huskies—yes, we’re proud adopters of these beautiful, spirited companions. Healthcare has always been my calling, and I’m honored to walk alongside each of my patients on their journey to better health.
