Initial Comprehensive Evaluation - 90–120 minute in-depth assessment including chart review, client/staff interviews, and written recommendations. - $250

Follow-Up Case Management - Ongoing support, care coordination, medication reconciliation, and provider liaison services.- $100/hour

Care Planning & Advocacy Reports - Custom written care plans or advocacy summaries for family or guardianship review. - $100/report

Clinical review, medical advocacy, and documentation support tailored for legal or ethical decision-making. - $150/hour

Care Compliance Monitoring - Periodic in-person or virtual review of plan of care adherence, environment safety, med compliance. - $100/visit

Guardianship Onboarding Assessment - Comprehensive intake for new wards including full medical history, current issues, and care plan. - $300

End-of-Life Planning Consultation - Coordination with hospice, advance directives, and goals of care documentation. - $150/hour

Medical Appointment Accompaniment - In-person or virtual support to advocate for client at medical visits (travel time billed separately, will not transport clients). - $125/hour

Medication Safety Review & Deprescribing - Full reconciliation, risk-benefit analysis, and deprescribing recommendations. - $150/assessment

Level-Up APRN is open to discussing rates on a case-by-case basis for those on a fixed income (Medicare, SSI, VA Disability) or under guardianship-supported care. Not all clients will be eligible for these rates.

Travel Time: $50/hour for travel exceeding 20 miles round-trip.

After-Hours Services: Additional 25% fee for services rendered outside of M–F 8 AM–6 PM.

Cancellation Policy: Cancellations made with less than 24 hours' notice may incur a $50 fee.
